The Kapiolani Community College Human Resources, in partnership with the Times Pharmacy, is organizing its annual 2021 Flu Shot Clinic. It is a two-day event; see below for dates and times:
- Day 1: Wednesday, October 6, 2021, from 12:30 pm to 3 pm
- Day 2: Thursday, October 7, 2021, from 12:30 pm to 3 pm
For every 30 minutes, 15 spots are available. If all of the available spots are taken, you will be placed on a waiting list.
Insurance: Times Pharmacy accepts both HMSA and Kaiser insurance and the standard flu vaccine and Fluzone High-Doze are 100% covered for both. Moreover, all other major insurance providers such as HMA, HMAA, UHA, Tricare, PSWA, Hawaii Laborers Union, Medicare, and Quest are accepted.
Fluzone High-Dose: Fluzone High-Dose Quadrivalent is a four-component (quadrivalent) flu vaccine approved for people 65 years and older. In the United States, Fluzone High-Dose Quadrivalent is licensed only for people 65 years and older. You will get an option to select Fluzone High-Dose while completing this registration. Without any insurance, the flu vaccine is $55 and Fluzone High-Dose is $75.
Please bring the following with you:
- Unexpired government-issued photo ID
- Insurance Card (e-copy is acceptable)
- Completed Vaccine Consent Form (To download click here. Please complete this form, print it, and bring it along with you.)
- Completed Pre-Vaccination Form (To download click here. Please complete this form on the day of your appointment, print it, and bring it along with you.)
- LumiSight UH green “You may report to campus” status (uh.campus.lumisight.com/)
Important note: You need a green “You may report to campus” status on LumiSight UH before arriving at the campus for your Flu Shot appointment. A staff member will be checking your green status on the LumiSight UH app/website. (Link: uh.campus.lumisight.com) On the day of the Flu Shot Clinic, all staff members including Times Pharmacy staff will have a green “You may report to campus” status on LumiSight UH.
Additionally, the Times Pharmacy will be requiring a temperature check upon form submission.
